Pinnacle Body Movement – will hold personal information on clients to include:

Name, Address, Telephone numbers, Email address…. for the purposes of contacting clients regarding their treatment and home care plan.
Information on type of treatment and location of injury/pain and symptoms/history associated with the injury/pain will be collected for purposes of continuity of care and to track progress/regression.

Information shall only be shared with the clients permission to another healthcare provider whom the client and I have deemed vital to be included in the care of the client working towards a common goal. Any other healthcare worker requesting information in regards to any of Pinnacle Body Movement clients shall not be given without clear permission from the client themselves.

Personal information shall be kept for 7 years following the last treatment and, in the case of minors, will be kept for 7 years following their 18th birthday if they are no longer under the care of Pinnacle Body Movement.

Clients can exercise their Individual Rights at any time without charge. However, if requests are considered repetitive, unfounded or excessive a reasonable administration fee can be charged.

All reasonable care will be taken to safeguard the privacy of all clients personal data by Pinnacle Body Movement.

Cancellation Policy

In accordance with standard practice, we regret that a charge will be made for missed appointments or non-attendance unless 48 hour notice is given.
